The reciever and giver portions on the certificate will be left blank for you to freely personalize and fill out how you would like. Everything else on the certificate will come prefilled (ex: date of purchase, owner signature, amount of tattoo, and tattoo artist selection - including the Sorry Mom seal).
Gift Certificate
When recieved, please do not lose this certificate. To redeem your Tattoo Certificate please submit a “tattoo inquiry” form on our website www.sorrymomclub.com and specify you are making an appointment for a “tattoo gift certificate”. Gift certificate must be physically presented at the time of the appointment before the tattoo procedure, to verify authenticity, or it cannot be redeemed. Tattoo will be worth the amount that is gifted and stated in this certificate. Tattoo must be redeemed within One Year (365 days) from the date this gift certificate was purchased. Tattoo will be done by the artist chosen at purchase, and cannot be changed. We can’t wait to tattoo you! & Tell your mom we are Sorry!
